SUMMARY
The MDS Coordinator is responsible for the timely and accurate completion of the Federal and State assessment tool. Position serves as an active member on the interdisciplinary and management team.
Job Knowledge and Role Responsibilities:
Follows established philosophy and objectives, policy and procedures of nursing service and contributes to change when necessary.
Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning delivery of care.
Demonstrates knowledge of the MDS rules and regulations as described in the MDS User’s Manual, including item coding, RAPS, care planning, electronic submission, scheduling, etc.
Have a working knowledge of PPS and the relationship to the MDS.
Follows established procedures, systems and utilizes approved forms to manage the MDS and PPS
Maintains Tracking Tools for the MDS and Medicare.
Make skilled services decisions and initiate Medicare non-coverage letters.
Demonstrates knowledge, skills, and techniques necessary to care for residents with the following needs: alzheimers, dementia, dialysis, IV therapy, infectious disease s, wound care, and all disease processes if applicable.
Provides nursing care in accordance with Resident Care Policies and Procedures and ensures the safety and well-being of the residents is maintained.
Maintains and updates license in compliance with State regulations.
Acts appropriately under the direction of the assigned supervisor and acts as an active member of the interdisciplinary team.
Demonstrates ability to adjust to changes in unit/shift assignments to meet resident and facility needs.
Responds appropriately to behavioral, medical, and fire safety situations.
Demonstrates responsibility for units assigned personnel and ensures quality nursing care is provided.
Aware of Resident Abuse Reporting and Prevention Policies.
Demonstrates knowledge of and complies with the Code of Conduct and Corporate Compliance Program when performing work functions.
B. Administrative Responsibilities:
1. Reviews medical records, resident care plans, consultant recommendations and staff assignments.
2. Provides nursing care on the unit when staffing necessitates and cooperates with other members of the medical care team in coordinating residents’ total needs.
3. Identifies and studies nursing service problems and assists in their solutions.
4. Observes nursing care and visits residents to ensure that nursing care is carried out as directed and treatments and medications are administered in accordance with physician’s instructions. Notifies supervisor when discrepancies are noted.
5. Demonstrates working knowledge of computer program, is able to input, correct, and print.
6. Demonstrates knowledge of the MDS and Case Mix Categories.
7. Understands the importance of the MDS User’s Manual.
8. Directs, prepares, and maintains the necessary resident records, both clinical and state mandated, according to required time frames.
9. Assesses the health records for each resident and ensures that written notations are made promptly of significant changes affecting:
a. Level of activity
b. Eating habits
c. Physical, mental, and emotional status
10. Completes the following duties:
a. Completes all MDS assessments for nursing
b. Participates in planning and adjustment of Medicare assessment
scheduling
c. Validates the entire MDS once printed
d. Writes RAP summaries
e. Assists with / teaches care planning process to nurses and other members of the team
f. Completes Medicare screen on admission
g. Reviews Resident Roster and Census
h. Reviews laboratory and x-ray reports
i. Attends meeting of supervisory and administrative staff
j. Assists with studies related to improvements of nursing care
k. Monitors documentation in the medical record for accuracy and notified supervisor.
l. Supplies CNA ADL sheets prior to the Assessment Reference date window and monitors for completion.
m. Demonstrates knowledge of and complies with the Code of Conduct and Corporate Compliance Program when performing work functions.
n. Is an active participant on the Quality Improvement Team.
C. Role Responsibilities – Nursing / Rehab Systems:
1. Demonstrates knowledge of each resident’s historical and diagnostic data in relation to its impact on nursing care.
2. Observes and assesses the total resident focusing on all body systems, skin, integrity, weight, and hydration status and reports changes in resident condition promptly to the Supervisor.
3. Gathers baseline information to identify potential and acute problems and thoroughly and accurately completes all areas of the MDS nursing admission assessment and quarterly assessment and Medicare assessments using currently approved format.
4. Demonstrates knowledge of each resident’s therapy schedule and those persons responsible for implementation and management of those services.
5. Prepares for and attends team and family conferences in a professional, supportive manner as assigned.
6. Communicates with nursing and clinical personnel on the implementation of the resident care plan.
D. Role Responsibilities – Interpersonal Skills:
1. Is aware of, and adheres to, Resident’s Bill of Rights and Confidentiality of Resident Information.
2. Interacts with residents and family members, co-workers, clinical and ancillary staff in a non-judgmental, supportive and calm manner.
3. Addresses family satisfaction issues immediately by reporting them to the Supervisor in a professional manner.
4. Is aware of Resident Abuse Reporting Law.
5. Participates as assigned in the orientation of new staff and demonstrates a positive, helpful, and enthusiastic attitude.
D. Role Responsibilities – Safety:
1. Understands and can demonstrate knowledge of role in Fire and Disaster drills.
2. Demonstrates ability to locate emergency equipment (i.e. emergency oxygen tank).
3. Demonstrates ability to identify / respond appropriately to potential behavioral outbursts.
4. Uses facility equipment safely. Understands how to use lifts appropriately and practices good transfer techniques.
5. Recognizes, removes, and/or reports potential hazards.
6. Understands OSHA recommendations.
7. Is knowledgeable of MSDS labels and hazardous chemicals.
E. Resident Care Responsibilities – Infection Control:
1. Washes hands between resident contacts as appropriate.
2. Demonstrates knowledge of and implements Standard Precautions.
3. Handles linens and disposes of waste properly.
4. Demonstrates ability to care for residents in precautions.
5. Demonstrates knowledge of correct positioning / care of catheters and drainage.
6. Understands how to handle infectious waste.
F. Resident Care Responsibilities – Resident Dignity:
1. Addresses residents in a respectful manner.
2. Discusses confidential resident information in appropriate areas only.
3. Understands HIPAA regulations and maintaining confidentiality of medical information.
4. Knocks on door prior to entering resident rooms, and maintains resident privacy and dignity when providing treatments or other resident care.
G. Other Duties As Assigned by the Assistant Director of Nursing / Director of Nursing:

“Occasionally" equals approximately 1% to 33% of the workday, “Frequently" equals approximately 34% to 66% of the workday, and “Continuously" equals approximately 67% to 100% of the workday (Typical full-time workday is 7.5 hours).
